These forms are only viewed by members of the My Pilates Fix team who are directly responsible for your care.

If you are booking a Pilates course via this website or over the telephone with us, or you do Pilates classes or private appointments with us, we use a company called TeamUp to manage the bookings and to store Enrolment forms.
